I believe Checkbox is a great tool if you're not in a hurry. The surveys are well-designed and the data collection is top-notch, but the process of creating a survey is incredibly slow, to the point of being frustrating. My university bought this software and we are required to use it instead of Survey Monkey. The vendor responded to my feedback, stating that the latest version of Checkbox should be faster due to improvements in caching. They suggested that if I'm still facing performance issues, it could be because my self-hosted installation doesn't have sufficient server resources for my database size. They advised me to reach out to their support team for guidance on potential adjustments or server setup recommendations.

Overall, I've had a positive experience with Checkbox. The support has been good and they're releasing a new version that may provide a better interface and faster response. It does what it promises, so I'm satisfied so far. The cost of Checkbox was a significant factor compared to other survey software. Some are quite expensive, so if you're unsure about the responses and results you'll get, it's smart to make a smaller investment. It has the same features as others, but was several thousand dollars cheaper annually. The web interface is a bit clunky and can be slow. Also, the security setup is strange, as you set it up on a folder, then an individual survey. It's not the most intuitive interface, but once you get used to it, it gets the job done. If cost is a crucial factor, I would recommend checking it out.
